The Plant Risk Assessment Form can be used as a template to record Risk Assessments carried out for the use of specific plant (including machinery and equipment) at the workplace / site.
It is a requirement of the harmonised WHS legislation to conduct Risk Assessments for certain Plant and/or equipment and/or workplaces where plant (such as mobile plant) is used. Refer to the legislation for information.
The Plant Risk Assessment Form includes sections for: scope, plant description and use, High Risk Construction Work, hazard and risk identification, assessed risk level, environmental hazards and risks, emergency response procedure, consultation, worker training, monitoring & review, worker and management sign offs.
