The Incident Report Form can be used to formally report incidents that occur at a workplace and to record any corrective actions undertaken.
Completed Incident Report forms must be submitted to the business owner, and if required, the business owner shall report the incident to the safety regulator in their State of operation.
It is a legislated requirement of the harmonised WHS legislation to report incidents, in particular, Notifiable Incidents at a workplace.
